HSPLS Passport Acceptance Information

LIBRARY ACCEPTANCE FACILITIES DO NOT PROCESS DS-82 PASSPORT RENEWALS, DS-4085  ADDITIONAL VISA PAGES, AND
DS 5504  NAME CHANGE AND BOOK REPLACEMENT
TO SUBMIT A PASSPORT APPLICATION IN PERSON AT A PARTICIPATING PUBLIC LIBRARY, YOU MUST HAVE THE FOLLOWING FIVE (5) ITEMS:
1. APPLICATION FOR U.S. PASSPORT, FORM DS-11
To download an application form, visit:

Or, forms can be obtained from a library acceptance facility.
Select which document (or documents) for which you are applying.
NOTE:  Passport cards cannot be used to travel by air.
All applicants must appear in person.
NOTE:  Please do NOT sign the DS-11 application form until the Passport Acceptance Agent instructs you to do so.
2. PROOF OF U.S. CITIZENSHIP
NOTE:  Original documentation submitted as evidence of U.S. Citizenship will be returned with the issued passport.
You may prove U.S. Citizenship with any one of the following original documents:
Ø Certified birth certificate issued by the city, county or state
Ø Consular Report of Birth Abroad or Certification of Birth
Ø Naturalization Certificate
Ø Certificate of Citizenship
NOTE:  Birth abstracts from California and Texas, and hospital birth certificates from any state will not be accepted.

3. PROOF OF IDENTITY
You may prove your identity with any one of these:
Ø Naturalization Certificate (not more than 10 years from issue date)
Ø Current, valid:
● Driver's license
● Government ID: city, state or federal (employee ID badges will not be accepted)
● Military ID: military and dependents
NOTE:  Your Social Security Card does NOT prove your identity.

FOR MINORS AGES 15 AND UNDER:
Both parents must provide proof of identity and bring the child's birth certificate in addition to the child's old passport, if any.
If one parent or legal guardian is claiming sole custody of the minor child, proof of custody is needed.
Ø That non-applying parent cannot appear before the Passport Acceptance Agent to show  their consent to the passport issuance, OR
Ø The consent of the non-applying parent or guardian cannot be located and obtained.

FOR MINORS AGES 16 AND OVER:
Ø    If the minor child does not have acceptable identification of their own, a parent, with acceptable identification, needs to accompany their child and co-sign the application.
Ø Only one parent needs to be present.

4. PROVIDE TWO (2) PASSPORT PHOTOS
Your photographs must be:
Ø Two color copies
Ø Identical 2 x 2 inches in size
Ø   Taken within the past 6 months, showing current appearance

NOTE:  Vending machine photos are not acceptable.
5. TWO (2) PERSONAL CHECK PAYMENTS
Check #1 - $25.00, payable to State of Hawaii - Director of Finance
Check #2 - payable to U.S. Department of State, with the following amount:
Ø
$75.00 (passport book) and $20 (passport card) - age 16 and over OR
Ø   $60.00 (passport book) and $10 (passport card) - age 15 and under
Ø For Expedited Processing, add an additional $60.00, with the notation "Expedite Fee."

PASSPORT PROCESSING TIME

Regular Processing: Eight (8) Weeks
Expedited Service: Three (3) Weeks
